Chronic Disease Management: Through Physical Activity

Living with a long-term condition can make staying active challenging, but regular, supported physical activity plays a key role in maintaining health and independence. Our Chronic Disease Management (CDM) Exercise Programme offers a safe way for people in the community to move with confidence.

What Is the Programme?

The programme runs for 12 weeks and includes two supervised sessions each week in a friendly group setting. Classes combine aerobic exercise, strength training, balance, mobility and flexibility, with seated and standing options available to suit different ability levels. Education and social connection are also central to the programme, helping participants build healthy habits that last beyond the programme.

Who Is It For?

The CDM programme is open to individuals living with long-term conditions who are referred by a healthcare professional. This includes people managing diabetes, cardiovascular disease, arthritis, neurological and pulmonary conditions, those recovering from cancer, and individuals at risk of deconditioning, falls or social isolation.

Why Is It Important?

The programme helps participants boost confidence, independence and quality of life. It focuses on improving strength, balance and mobility, supporting pain management and enhancing mental wellbeing.

How to Access the Programme

Participants complete health screening and physical assessments before starting. Sessions are delivered by highly qualified instructors with experience working with chronic conditions. The programme is free to participants and delivered in community locations, making it an accessible physical activity pathway within healthcare services. Referrals are made by healthcare professionals. Participants attend an induction session before beginning the programme.
